REGISTRATION INCLUSIONS
Professional Development Seminar – 5 & 6 October 2006
The PDS is for Registered Music Therapists and includes attendance at all PDS sessions, morning and afternoon refreshments, buffet luncheons, PDS program and name tag. Registration between 8.00am – 9.00am both days.
Conference – 7 & 8 October 2006
Registration includes attendance at all conference sessions, morning and afternoon refreshments, buffet luncheons, Graduate Registration Ceremony ticket, delegate satchel, name tag and conference program. Registration between 8.00am – 9.00am both days.
DAY REGISTRATION
Day registration includes attendance at all Conference and/or PDS sessions on the day of attendance, morning and afternoon refreshments, buffet luncheon, name tag and conference program. Registration between 8.00am – 9.00am both days. (Please note that the Graduate Registration Ticket is not included but may be purchased separately - $25.00 per person)
GRADUATE REGISTRATION CEREMONY - Saturday 7 October at 5.00pm
Come along to welcome our new graduates into the profession. Family members and friends welcome. Light refreshments will be served. The Graduate Registration Ceremony Ticket is included in the registration fee. If you require extra tickets please indicate when completing your registration form. Extra tickets $25.00 per person.
ACCOMMODATION
Accommodation has been secured at a highly competitive rate for delegates at the Travelodge Hotel, Phillip Street, Sydney.
(5 min walk to the State Library of NSW). Standard Rooms $145 per room per night – single/double/twin - include kitchenette, fridge, microwave. A third person can be accommodated at $16.00 extra per night. A deposit of the first night's room fee must be included in your registration. Accommodation cannot be booked unless a deposit is received. Please note cancellation policy in general information.
STUDENT REGISTRATION
A student is defined as a holder of a student identification card from a recognised tertiary or secondary educational institution. Your student identification number is required when you submit your registration form. Without this information, the full registration fee will be charged.
CANCELLATION AND REFUND POLICY
Cancellations must be notified in writing to the Conference Secretariat - CLEMS by email amta2006@clems.com.au
Cancellations before 5 September 2006 will incur a $100.00 cancellation fee. Cancellations after 5 September 2006
cannot be accepted, however transfer of registration to another person will be accepted. All refunds will be paid at the conclusion of the conference.
HOTEL CANCELLATION POLICY
Any change to a reservation must be notified in writing to the Conference Secretariat, CLEMS and not directly to the hotel.
Unless cancellation notification is received in writing 7 days prior to arrival, the deposit will be forfeited in all instances as per Hotel Policy. A Tax Invoice will be issued by the hotel on check-out.
ACKNOWLEDGEMENTS
Your registration and payment will be acknowledged in writing with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form. The registration form is an approved Tax Invoice by the Australian Tax Office.
INSURANCE
Participants carry their own risk for personal injury or loss of property, including baggage, during the conference. We strongly recommend that, at the time of booking your travel and tours, you take out a travel insurance policy. This policy should take into account loss of deposit through cancellation, medical insurance, loss or damage to personal property, and financial loss incurred through disruption to accommodation or travel arrangements due to business failures, strikes, or other industrial action. The organisers and the conference secretariat are in no way responsible for any claims concerning insurance.
PAYMENT METHODS
Full payment should accompany Registration Forms. Payment may be made by personal cheque, money order payable to the AMTA Conference. Please print delegate name and organization on back and forward to the Conference Secretariat. Visa and Mastercard are accepted. All prices shown are inclusive of GST. Please note all transactions on your credit card, via the Secure On-line Payment System, appear on your statement as MIE Software Pty Ltd Port Melbourne.
PRIVACY ACT
The Privacy Act 2001 provides that, before your name, organisation and state/country details can be published in the list of the conference delegates for distribution to fellow delegates or any other party, you must give your consent. If you DO NOT wish to have your name and details included in the Delegate List please indicate this on the registration form.
DISCLAIMERS
The AMTA Organising Committee including the Conference Secretariat, CLEMS, will not accept liability for damage of any nature sustained by participants, accompanying persons or guests for loss or damage to their personal property as a result of the conference, professional development seminar and related events.
